Thank you for your interest in owning a Sage Poodle!
If you would like to own one of our puppies, please begin the process by reading our purchase agreement and filling out the questionnaire below.
We always try to find the best possible home and environment for our puppies. Due to various reasons, not all applicants are a good fit, and some are not selected.
Please read our purchase contract and make sure you are willing to abide by the terms before filling out the questionnaire below.

1. Why did you choose a standard poodle? Are you purchasing this puppy/dog as a gift for someone?
2. Describe your household (family members and their ages) and any other pets you currently own. (species, gender, age, neutered or spayed, etc.)
3. Does everyone in your household want a puppy/dog?
4. Is anyone in your household allergic to dogs?
5. What age dog are you interested in obtaining?
6. What is the timeline for when you would like to receive your dog? (ASAP, in a few months, next year, etc.)
7. What gender of dog are you hoping for? (male, female, or either) Also, why is this your preference?
8. Do you have a colour preference? If so, what colour?
9. Do you have a preference regarding tail docking or dewclaw removal? Currently, the Canadian Veterinary Association. does not recommend cosmetic surgery on pups.
10. Have you ever owned a puppy/dog before? If so, what happened to the puppy/dog?
11. What is your experience with dogs and training? Do you have a trainer or puppy class that you plan on using? Poodles are smart and require training. An untrained dog is not fun for anyone!

13. Are you aware of the temperament of this breed?
14. Are you aware of the grooming requirements for this breed? Most groomers would like to see a poodle once a month, and it can cost $100 each time. Lack of proper grooming can cause skin problems. Are you committed to this necessary expense?
15. How often do you plan on bathing? Brushing? Brushing teeth? Taking the puppy/dog to the groomers?
16. Are you aware of the health issues that may affect this breed?
17. What food do you plan on feeding your puppy/dog?
18. What activities will your poodle be involved in? (companion, shows, breeding, therapy/service, working, hunting, etc.)
19. Please tell us more about your plans for the puppy/dog.
20. Please describe your expectations for your poodle puppy/dog. This can include coat length, personality traits, energy level, etc.) We always want to make sure we have the best fit for our puppy homes.
21. Tell us a little about yourself. (occupation, family, hobbies, etc.)
22. Where will the poodle live? Where will your poodle stay during the day? If working from outside the home, how do you intend to care for the puppy while at work? Where will your poodle spend the night?
23. What is the longest the puppy/dog will be alone? When you are not home, how will you keep your puppy safe? Where will the dog be when you are unable to supervise?
24. Do you agree that this puppy will not live outdoors? Poodles are indoor dogs. They do not get a second winter coat as many other breeds do. They also do not do well when left alone for too long. They are happiest when they are by your side.
25. Do you already have a trusted veterinarian? If so, please provide veterinarian contact information.
26. Tell us about where you live. Are you in a city or rural?
27. Do you live in a house or an apartment? Do you own or rent? If you rent, are you allowed to have a dog? (If you rent, please provide the landlord's contact info.)

Part of the sales agreement is that the puppy will be sold on a CKC limited registration and a non-breeding contract unless otherwise agreed upon.
30. Do you agree that the puppy/dog shall not be bred? Will you agree to spay/neuter the puppy by the time the puppy is two years old? Earlier is advised by most veterinarians.
Also, part of the sales agreement is that the puppy/dog will not be sold, transferred, or given to any individual or any retail or wholesale establishment for sale or resale to the public, nor to any research institute where medical experimentation or other practices take place. This puppy/dog is NOT to be placed in a shelter or humane society or turned over to a rescue organization.
If, for any reason, the buyer is unable to keep or care for the puppy/dog, the dog/puppy is to come back to us.
Also, part of the sales agreement is that the buyer will provide the puppy/dog with veterinarian care upon sickness, disease, or injury, and shall take the puppy/dog to a veterinarian at least once a year for an annual health examination and routine vaccinations.

32. Our puppies leave when they are 8 - 9 weeks old. Are you able to take the puppy home within 2-3 days of that set date?
33. Do you agree to give us annual follow-up reports and photos? It helps our breeding program to see how the dogs turn out. I also like to hear how they are doing.
34. How did you hear about us?
35. We may not have a litter at this time. Are you willing to be placed on a waitlist?
36. Are you willing to place a nonrefundable, but transferable holding fee? We only accept deposits once we know we have a puppy for you.
37. Are you willing to sign a legally binding contract agreement to our sales conditions? The contract is provided after we know we have an available puppy for you. The deposit to hold a puppy is accepted only after the contract is agreed upon and signed.
